The association between adherence to iron supplementation and hemoglobin status among pregnant women

Background: Anemia in pregnant women remains a health problem in Indonesia and may increase the risk of pregnancy complications. Iron supplementation is an important preventive measure, but its effectiveness largely depends on adherence.
Objective: To determine adherence to iron supplementation, Hb status, and the relationship between adherence and Hb status among pregnant women in Songgom District, Brebes Regency.
Methods: Observational study used a cross-sectional design involving 132 pregnant women selected through proportionate stratified random sampling (PSRS). Adherence was measured using the MARS-5, while Hb status was assessed using an Hb meter and KIA book records. Data were analyzed using the Chi-Square test at a significance level of p<0.05.
Results: Most respondents were aged 20–35 years and had primary-level education, with moderate adherence to iron tablet consumption. Overall, 90.9% were non-anemic and 9.1% were anemic. A significant relationship was found between iron tablet adherence and Hb status (p<0.05).
Conclusion: Iron tablet adherence was significantly associated with Hb status among pregnant women in Songgom District, Brebes Regency. Education, monitoring, and support from healthcare workers and families are needed to improve adherence and prevent anemia.
